Re: An old friend needs prayer

The latest from Maggi

Hi all, thank you for all your good wishes.

I was very poorly after the first biopsy, with excruciating pain and continual vomiting. Doc finally got it under control by doubling my already hefty morphine dose, and throwing an additional anti-sickness med into the mix. In the midst of all the pain and puking, they called to say the biopsy had failed and would need to be repeated...

The repeat biopsy was done on Tuesday, and we are praying they got a useable sample this time... not sure I could face the procedure again. I am no wimp when it comes to needles... being diabetic, I have to inject myself abdominally twice a day... but the biopsy really hurts. Even the local anaesthetic is painful, then they make an incision and shove the equivalent of a sharpened knitting needle through your entire midsection, before snipping out a few lumps of tissue, wiggling the needle around before each snip. You have to be awake throughout, as patient co-operation is required, and the anaesthetic only really affects the surface area... you feel everything that is going on deep down inside.

The internal bruising comes out over the next few days, so the pain and nausea has almost reached its peak. Thanks to the increased meds, I am not in quite so bad a state as last time (no vomiting so far, though I am very nauseous); but the morphine dose is now at sedative level, so I am sleeping rather more than I would like with so little time left.

the latest from Maggie

I got the results of the second biopsy this afternoon - it is a ductal adenocarcinoma, as expected.

Standard surgery was ruled out from day one, but I was shocked to be told today that, because of the size of the tumour, chemo may not even be a viable option. I am seeing the oncologist on Tuesday to find out where we go from here. Dr Mukherjee is supposed to be one of the best oncologists in the country, according to my surgeon.

My hopes were pinned on a rare surgical procedure that involves removing the portal vein and replacing it with a graft. It has been successful in some cases where there is no metastasis to the liver/lungs. Unfortunately, I learned today that it is usually only considered in tumours below 2cm (mine is 9cm). However, my surgeon has offered to try to arrange a referral to someone familiar with the procedure so that this avenue can at least be explored.

Time is fast running out. I need that miracle!

Re: An old friend needs prayer

It has been a week of highs and lows.

Had to call out the emergency doc early Saturday as I was in agony and had been hurling for 36 hours straight, again. After injecting me with diamorphine (heroin) and anti-emetics, he left to organise a nurse to come and fit a morphine pump into my arm. Before that could be arranged, I took a turn for the worse and an ambulance was despatched to collect me. However, the crew were not willing to move me in that state, so administered Entinox to take the edge off the pain whilst we waited for an IRV with paramedic to dose me up with more diamorphine and accompany me into hospital. She had to give me a third shot en route. On arrival, I was still unable to answer questions etc due to the pain and vomiting. In desperation, I asked for some Diclofenac to add to the morphine, paracetamol, hyoscine etc... my GP had refused to prescribe this for me any longer, despite its effectiveness, because a study had linked it to osteoporosis in later life; but as I pointed out, that is the least of my concerns now... and it worked!

The pain and sickness subsided immediately, and I was given IV fluids and a sleeping pill so I got some rest at last, then I was able to come home on Sunday. The Diclofenac is now part of my regime, and means that I get a couple of short periods each day when I am effectively pain free and can function almost as normal. Unfortunately, I am still in a lot of pain in between, and I am already on the maximum dosage of Diclofenac...

On Tuesday, I met with my oncologist for the first time. Dr Mukherjee is reputed to be one of the best in his field, and I took to him immediately. The news was better than I had anticipated.

He told me that my tumour is around four times the size of most newly-diagnosed pancreatic adenocarcinomas, hence the very pessimistic attitude I had encountered thus far. However, the evidence suggests that it is entirely localized, which is all but unheard of, and which gives cause for cautious optimism. He thinks I might have another 10-12 months, which is considerably longer than all previous estimates... at one point, it was suggested that I might not see Christmas.

Normally when pain is an issue, chemotherapy is combined with radiotherapy. In my case, however, the tumour is so massive that the combined dosage required would be toxic in itself. Therefore, he is proposing to start the chemo, and to feed in radiation cycles separately as required. I have my pre-chemo assessment on Friday of next week (11th November), then my first chemo session the following Tuesday (15th November).

Please keep up the prayers/positive thoughts... I know they are helping.

Re: An old friend needs prayer

Latest news form Maggie for all those who are not in her mailing list

Hi All

After a week on Diclofenac, I was actually able to go out for the day on Saturday - a huge change from the hospitalisation hell of the previous weekend!

We took a trip to Norfolk... ate chips on the sea front, bought lavender linen spray for my sheets, and drank and was sprinkled with healing waters from the Holy well at the Anglican Shrine at Walsingham. All in all, a lovely, peaceful day. On the way home, I popped into Argos and bought a Yantra mat.

The mat and the linen spray are helping greatly with sleep issues... eating is still a big problem, though. I have no appetite, and feel nauseous at the very thought of food, but I am told that it is desperately important that nutrition is maintained throughout the chemo. This is a worry.

I have heard back from my expert friend about the chemo... it appears that I am being offered only the most basic treatment, and there are two combination therapies available in the UK that would offer me much better prospects. I have taken this up with my oncologist as a matter of urgency, as I have my pre-chemo assessment tomorrow and my chemo is supposed to start on Tuesday. Please pray/cross fingers/send positive thoughts that all goes well and he responds favourably.

Much love
