Have a problem after installation

I installed Phoca Download successfully and created Phoca Download tables manually. And everything is OK, but then I try to add the new download file I see the next mistake

P.S. I have another problem: when I installed Phoca Download, the Joomla says me

Folder phocadownload creation failed! Please create it manually.
Folder phocadownload/userupload creation failed! Please create it manually.

I can understand where on my server I should create Folder phocadownload and Folder phocadownload/userupload. Please tell me!

Hi, you should enable debug mode, seems like you have problems with database (e.g. not all tables or columns are created, etc.)
