Unreachable Error

Hi, I am receiving the URL is unreachable error while using your facebook comments module.
joomla 2.5.4
rockettheme template, visage
have searched thoroughly in your forum and online for any tests to run - none have stopped the warning.
have checked, rechecked, and checked 10 more times the app domain and site url on facebook = i believe they are correct.
had my site offline and thought was surely the cause - but even when i put the site back up, still have the error.
have tried clearing all my cache just for the heck of it.
have installed phoca open graph and site plugin = no change.
not sure what else to do - please help.
