I just so happened to need one last week for a last minute gift and I whipped up my own holder. Then I thought it'd be a great little project to share so you get the template too! It's really simple, took about an hour to decorate, print, cut and fold. I started a few different themes so I'll have them ready to grab 'n' go when I need them later.
