Make sure you're using PHP 4.11 installed via the auto installer (make a backup of your c:winntPHP.INI file)

It will ask you if you want to configure for IIS. Answer yes. You'll have to reconfigure your PHP.INI and re-add the INCLUDE PATH line per the CACTI instructions.

Also, the paths are very important. Read the Win2000 docs and make sure you're using the proper slashes. None of the paths should end with a slash.

Finally, get the free MYSQLfront at http://www.mysqlfront.de and connect to the CACTI database. Under the table entitled "Settings", make sure the data fields for global_auth and global are empty.
That will allow you to bypass any security issues initially.

I'm running it under 2000 and IIS. Read the instructions carefully and it will work.
