mysql stats 2.0 php script not running?

I just downloaded the addin for mysql to monitor the status of the server and it really seems cool. the problem is that ican't get it to work!

this is me trying to execute it manually...

i get no data output and here is my cacti and poller logs

does anyone have any ideas on what could be causing it not to work? i've already issused the grant process rights to my cactiuser account.
