I have seen some posts related to this, but not sure if this was directly addressed. Anyway, here is how we did it....

Screenshot, showing networks in two different regions, same IP, same interface, different data coming from different Cacti instances (names changed to protect the client).

Just did some more work on this. Here is a working demo in the cloud you can log in and see. We also have some massive scale monitoring going on inside this box, so may take a few seconds for graphs to render:
