Percona mysql template different port

Hi All,
I am new to the forums and I am hoping to get some basic help on setting up a template.

So I am using the ss_get_mysql_stats.php script for, I believe, all of my mysql templates. Some of our instances don't use the standard 3306 port for mysql and some instances have more then one port. Basically I want to report on the below format

Server A: Port 3306
Server A: Port 3406
Server B: Port 3406
Server C: Port 3306

So the problem is, since I am new, I am having a hard time understanding how to accomplish this easily. The closet I have seen is to add a new "Data Input Methods" and pass it in through the "Input String" box. The problem is that if I have to do that, then I am going to effectively double all my reports I have to choose from.
I am proficient in PHP and I don't mind modding the script, but I am hoping there is something I am overlooking because of how new I am to this. Any help would be appreciated.
