Need help with Graph Template

Hello everyone.
I have a custom Graph Template for monitoring my websites.
I have Custom Cata in my Data Template: hostname, URL, port.

Now there can be several sites monitored on a single host, each will have different URL (from my Custom Data, defined each time when creating the graph). How can I put the 'url' custom data into the graph name/title so that I could distinguish which graph is for which site?

I know something similar exists for Disks, Interfaces, where you can include mountpoint or interface description in the graph title. The variables are |query_somethinghere|. In fact I need that but just for my Custom Data: hostname and URL.

Any ideas?
