Cannot add new template (new Install)

Hi everyone - I'm not able to view 'select' the host after I installed the thold plugin. See the screenshot below. This is during the threshold creation wizard.
Can someone help?

Adding to this the URL is not appended with the values like hostid, graphid and dsid. But doing so manually gets me to create the threshold template. Eg,

1) http://192.168.107.133/plugins/thold/thold_add.php (Get to the template creation wizard)
2) http://192.168.107.133/plugins/thold/th ... ?hostid=11 (need to add the hostid to get cacti to select the host as my list of graph items don't show up after I select the host.
3) http://192.168.107.133/plugins/thold/th ... graphid=13 (again need to manually do this)
4) http://192.168.107.133/plugins/thold/th ... 3&dsid=113 (Do this to get my "Create" button
