"Monitor" Plugin and execution of scripts

Hi. I've installed Monitor Plugin v1.3-1. I have a network of devices I need to monitor the state (online or offline), but I want my Cacti Server to send a SNMP Trap to another server whenever some device goes down, and when it comes back up.

I believe the simplest way would be to make Monitor Plugin automatically run a shell script whenever it detects a state change, passing some arguments (IP of the device, type of alarm). Does anyone know if that's possible? Or how to accomplish this using another plugin?
