Data Source Template Not Triggering

I have attempted to build a custom template for my Mediant 1000 to collect Trunk performance stats, but can not seem to get the device to collect data.

When the the Data Source's Data Input Method is set to Get SNMP Data (Indexed) and I run the poller in debug I don't see the requests being made. If I flip the Data Input Method to Get SNMP Data I see the requests in the pollar with errors.

Also the graph template does not appear in the drop down for the "New Graphs" link. I am sure I am missing something obvious.

I am attaching my XML so hopefully someone will see what I have wrong.
