no create button when configuring "Thresholds"

I've got a problem that I cannot find out the "create" button when I plan to configure thresholds.

"management"-->"thredsholds"-->"add"--> i can select host and graph but there is no "create" button to proceed the creation. (please see the image I enclosed)

I am using cactiez which is developed base on cacti 0.8.7c (downloaded from by http://cactiez.cactiusers.org/).

Everything is working fine except this important feature. Expecting someone who is expertized in cacti can post some solution.
