NPC 2.0.4 GUI cannot display any data

my NPC do not display any data, as blew:

i have read all document here , but still cannot found the problem .
long_output has already added to all necessary tables .
cacti & Nagios all look fine.
/var/log/messages have some error "Apr 25 00:00:00 localhost ndo2db-3x: Error: queue send error."

/var/log/httpd/error_log have some error "PHP Notice: Undefined index: action in /var/www/html/cacti/plugins/npc/top_graph_header.php on line 95, referer: http://ipaddress/cacti/graph_view.php?a ... leaf_id=19"

the npc_hosts is empty .

NPC version:2.0.4
cacti verison:0.8.7i
nagios version:3.3.1
ndo2db.debug file looks ok ,not have error information.

pdo_mysql is enabled.

Hmm...
I had a same problem with you.
When i use ndoutils-1.4b9 instead of ndoutils-1.5.1,it's ok.
Please check your ndoutils version.
