Interface names with Nortel/Avaya 5600

I'm trying to get Cacti to use the Slot/Port notation to label the interfaces on an Avaya 5698 switch, but it uses some ifcXXX number instead, which I'm not even sure where it's getting. How can I tell Cacti to use the correct notation?

An example of what I'm talking about for port 1/13. This is how Cacti is labeling it:

|host_description| - Traffic - ifc13 (Slot: 1

The way I would want it labeled would be:

|host_description| - Traffic - 1/13

Or even this would be acceptable:

|host_description| - Traffic - Slot: 1 Port: 13

I'm sorry but I do not understand what this means. Are you saying I should create custom graph templates?
