Has anyone ran into any graph templates that import fine and you can modify them all day long but they don't show up in the list of "Associated Graph Templates" as part of the menu to add them to the host/device?
I have attached two screenshots showing that it does show up in the list of Graph Templates on the console but subsequently doesn't show up in the Graph Templates list menu on the host.
BTW, I am using version 0.8.7g on Ubuntu. Let me know if I can provide anything else.

Though I don't have any server installed HaProxy,
I guess, probably you have not imported original 'SNMP - HaProxy Statistics' template
before importing 'HaProxy Session Statistics' template.
After both installed, you can add data query (not graph template) like this: Graph template (you mentioned) is associated with this data query:
