I am unable to send test message via SMTP from my Cacti installation.
This is the message that I receive:
Unable to connect to SMTP Host '10.10.4.123': (13) Permission denied
What needs to be set up in order for Cacti to communicate via SMTP?

Check that your SMTP server allows relay requests from your Cacti box IP Address.
It's also possible that you may need SMTP Auth (unless it's Exchange). If it is Exchange, and you have added the Cacti IP into to allow relay list, ensure that you stop and restart the SMTP Virtual Server for the changes to take effect.

Threshold messages are working properly. It's just the test messages that are not working.

THOLD uses the same SETTINGS plugin as the test email message. My guess if that this isn't SMTP related at all, but a security or permissions issue.

That's what I thought, but I cannot find any information on this error message:
(13) Permission denied
What file permissions should I check?
