receiving in-accurate thresholds alerts

I set up a threshold on a disk space. if it goes below 10%, it should create syslog alert. I noticed that "threshold breached" alarm never came out first but "restored to normal" alarm came out. I'm using cacti version "0.8.7a" and thold version "0.3.8" for windows platform. This is the second time I've seen this. Is this a known problem? Is there any patch for this?

I think I have the exact same issue - the only difference is, that our notification goes via e-mail and we monitor the ping reply from a host. And we also never get a "threshold breached".
