The reason 'loopback0.gw4.phl1.alter.net' is not updated in the 'rrd_graph_items' table is because it was entered there as text. You can make this field always reflect the current host by editing the first field of the ping graph. Then change the 'Text Format' from 'loopback0.gw4.phl1.alter.net' to '<ip>'. Perhaps I will make it that way for the next version.
