No thresholds were created

after install the thold plug-in, when create the threshold template, it said "Error A template with that Data Source already exists!" and i fixed it by following the instruction in :

but when go click "Device" ==> select one of the host ==> "Create Graphs for this Host " ==> "Auto-create thresholds"
an error has been displayed : "No thresholds were created."

please help.
