How to specify the "Custom Data" in Data Sources using "Interface-Traffic Template"?
And what's the meaning of "Index Type", "Index Value", "Output Type ID"?

you're not suppose to type anything in. In the device's screen, click the link in the top right corner "create graphs for host". Then on the following page, select which interface(s) you want to graph. Select the graph type, then click create.
