Transfer the RRD-data from one interface to another

What are methods which allow to transfer the RRD-data from one interface to another.
On differents or on the same routers.
It is necessary at replacement of the equipment,
switching of the ports, at change ID's and other.

You first have to create the traffic graph on the new host. Then, copy/rename the old rrd file over the new one(s) you just created.

It's pretty simple. Just can't do it from the User Interface.
