Ah Windows, my old friend. Most likely this is because the way you specify the path doesn't exactly match the way Cacti stores it, so Weathermap can't figure out which DS is related to that rrd file, and so can't use poller_output.
Check in the debug log again, and there's a line from WEATHERMAP that says something like "Your rra_path is ..." make sure your path is specified *exactly* like that.

Weathermap 0.98a is out! & QuickTree 1.0. Superlinks is over there now (and built-in to Cacti 1.x).
Some Other Cacti tweaks, including strip-graphs, icons and snmp/netflow stuff.

so I'd suggest upgrading. I knew I went through this whole poller_output debugging thing with one user a while ago...

Looking further, 0.96 changelog also has the following new features:
"RRD datasource can take SET rrd_default_path to make configs a little easier to read.
RRD datasource can take SET rrd_default_in_ds and rrd_default_out_ds for non-Cacti users.
RRD datasource can get Cacti query information (in poller_output mode ONLY) - like ifAlias, ifSpeed etc
RRD datasource can take the ifSpeed/ifHighSpeed from the above, and use it in the map.
RRD datasource fills in Cacti cacti_path_rra and cacti_url with Cacti base path and URL

WOW! What a difference a few years makes. I like the changes you've made. It's going to take some getting used to all the enhancements.
I am still seeing "UNKNOWN" I even waited for more than 3 polling cycles just to very. Here's the pertinent information in the logs:
