Help understanding using a PHP script & graph output

Hi all

I'm new to all this so any help will be appretiated.

I have a php script I run which outputs the following:
C:\web\cacti\scripts>php c:\web\cacti\scripts\gameserver.php hl2 196.24.9.35 27015
X-Powered-By: PHP/4.4.2
Content-type: text/html

player_count:21 max_players:21
C:\

I'm trying to grab the player_count & max_player count values but dont understand how to do this. I dont understand how to tell cacti to read these vaules and to graph them. A simple explenation on how this is done would be greatly apprectiated.

Debuging gives me this:
