OK I am finished with my first revision. You can download it here:

http://www.raxnet.net/downloads/database-schema/

Feel free to use the provided 'phpMyAdmin.sql' to modify the provided PDF. My Cacti development database is named 'dev_cacti_2'.

For some reason I forgot to add foreign key information for the host* tables. This has been fixed and they have been added to the database diagram.

Ian, check out DeZign for Databases (http://www.datanamic.com/). It is a very inexpensive ERD tool. I've been using it for a couple of months after someone turned me on to it. It is a really, really, useful tool--all the way down to writing the sql scripts for table creation for you.

I also found a tool called TopStyle Pro (http://www.bradsoft.com/topstyle/) for creating, editing, managing style sheets. It has helped me tremendously in the projects I've used it on.
