I am using cacti to monitor my switches. When I open the graphs, at the top of each graph, it has "[host_description] - Traffic - ethernet1" and then continues like that for each port, all the way to ethernet48. What I would like to do is replace the variable that displays the ethernet port number with the actual name of the server that is connected to that port. This would make it much easier to read. Is there an easy way to change this variable? Where is it located? Thanks.

Thie variable is read from interface MIB using snmp. If your hosts can be found there, no problem. But to be honest, you'll won't find it there (if would at least request some reverse DNS lookup). A starting point would be something like CDP neighbour (for CISCOs, obviously) which would result in some IP addresses. Or you'll have to fille in some user configurable MIB fields that go along with the interface index ...
Result: no good solution up to now
