I've been beating on this thing with a rubber mallet for two full days now.

I finally have PHP5 happy with IIS6 (on W2k3 server)
I finally am getting RRD files
I am not getting images.

It seems that dynamic image-generation via rrdtool isn't working. I enabled debug mode and refreshed a browser pointing at the graphs to get some good output in the log file. If I cut'n'paste that into a cmd window on the server, it spits out PNG (which I have to capture via "> foo.png").

I'm using the latest supported versions of everything:
cacti-0.8.6g
rrdtool-cygwin-1.2.11
php-5.0.5
mysql-4.1.15
net-snmp-5.2.1.2-1.win32.exe

Underlying versions are:
W2k3 Server
IIS v6(?)

I tried this with IE and firefox, so it isn't a browser issue.

(fwiw, I'm a Solaris admin, I have assorted reasons to make this work on win32 :) )

Try setting up the various permissions I've outlined in my updated windows guide first.

Ok, finally got it.

The server is in an AD domain and by default it will not search local accounts when setting security, so user IUSR_XXXX (where XXXX is machine name, of course) was not appearing as a valid account.
