Breaks in graph

Recently i installed cacti version 1.0.4 on RHEL7, Here i do have issue at the time of generating graphs.

1. Graph Breaks.
2. Live is not working for percona templates.
3.Unable to generate the file system graphs data(Using SNMP-mount points data query) on few RHEL6.9 servers.
