Graphing Nest Thermostat Data

Hello, I have been muddling around with this for a few days and I can not figure it out. I am a novice programmer and am working my way through this.

Here is what I have:
I am pulling in 3 values using the Nest API and a cron job every minute; temperature, humidity, and battery charge. and placing these values in a mysql database.
I have a php script that outputs the last value from the mysql db of each in this exact format every time it is called:

temp 68.3
humidity 36.0
battery 3.9

I have no idea how to get these values into cacti. I tried to follow these instructions http://forums.cacti.net/viewtopic.php?t=11288 but I get no graphs.

What i am envisioning is the cacti poller executes the php script and grabs those values and graphs them every minute. I just can not get there. Any help would be appreciated.

So... what I can not figure out is how to make those 3 values become inputs (output data sources??) for graphs.

Also in the log i get PHPSVR: Poller[0] WARNING: PHP Script File to be included, does not exist

but i have the php script in /usr/local/share/cacti/resource/script_server

I am runninug Ubuntu 16.04.1 LTS
