Discovery finds hosts but does not add to cacti

Fairly new to cacti. Been using over 3 months. First time having this issue. I can setup Discovery to find network hosts, run php findhosts.php -d -f and discover hosts but but get following output:

08/25/2015 08:37:03 AM - DISCOVER: Poller[0] 254 IPs Scanned, 32 IPs Responded to Ping, 4 Responded to SNMP, 0 Device Added, 0 Graphs Added to Cacti

for some reason I am unable to get the devices added. Any ideas would be fantastic.

Have you created Discovery Templates?
