Extreme Switch Interfaces not correctly identified

Hello, I am running a simple network comprising of extreme switches. I have the SNMP community strings configured correctly and cacti is correctly picking the basic information like contact, location and all. The problem is that it doesn't seem to correctly identify the interfaces as shown below.
