Creating new THold - unable to get host to apear in name

Hi - I have a number (1079) thresholds - most of them the name looks like this "zlabads24.uu.deere.com - Used Space - /dev/shm [hdd_used]" however, I added a new THold template today for memory utilization and the name looks like this "MemoryUtilizedDS [MemoryUtilizedDSI]".

Any idea on what I did wrong or what changed?
