Interface naming issues for Nexus 2000 FEX

My company is using Cacti to graph all of our interface statistics. We use a lot of Nexus 5000's with Nexus 2000 fabric extenders. We have multiple Cacti instances (one in each datacenter). However, something strange has happened with one of our instances. On the Nexus 5k, all interfaces have the correct name unless the interfaces belong to a Nexus 2k FEX.

I've done debug SNMP queries and all of the data is correct in SNMP. For some reason, Cacti is naming these graphs incorrectly. I'm 100% positive it's something I've done, but I've no idea what I could have done to cause this. FYI, this is a 0.8.8a instance running on Ubuntu LTS 12.04.

I am also having this problem with our FEXs connected to a Nexus 5548UP. I have deleted all of the interfaces and recreated them and I still get the results.
