thold not triggered (it's may hundredth same subject)

Good day everyone...
first of all, I would to inform that this is looks my first post, since I forgot my previous account (unfortunately unable to restore it).
And also this topic may hundredth subject with "thold not triggered" subject. Believe me that I've been trying to search the mentioned topic and yes I found many subject of mentioned issue, many suspected cause and many ways to resolve, I've tried to follow.. today, the problem still persist... I can't resolve it...
(I assume this is not in same root cause).

I enclose the detail on attached files.
(txt file is export result from mysql [thold_data], csv not allowed)

Cacti 0.8.7g
thold 0.4.9-3

Any advice is gratefully received.

enabling debug..

no error found in cacti log.
