How to get the data from Windows Performance Counters?

Hi, I am monitoring a few Windows servers using Cacti and SNMP, but it would be really neat to be able to monitor the performance counters. How can I achieve that?

I guess at the very least I would be able to install Zabbix agent on the servers and then write a script to query it from cacti, but maybe there is a better way (or maybe somebody already did that)?
