Hi,
There is a bug in v. 0.4.9-3 in thold_functions.php in line 1492.
There is:
logger($item['name'], 'warning', 0, $currentval, $trigger, $item['thold_fail_count'], $url);

and should be
logger($item['name'], 'ok', 0, $currentval, $trigger, $item['thold_fail_count'], $url);

Should be fixed in SVN 1972
