I did run one of the rrdtool commands as root perhaps that built some system wide font cache, running fc-list as _www returns the list as fast as any other user.
Re: rrdtool 1.4.3 + php 5.3.1 = Slow Graphing (rrdtool@100%
Just to add my experience - I was having the same problem in apparently identical circumstances. I followed the most recent posts regarding fc-cache and .fontconfig but this did not improve rendering times. The last post about running a rrdtool command as root did fix the problem. So apparently, rrdtool is able to do some caching as root that it is not able to do as _www.

Did you research SELinux in that direction?

I'm lost here.
I am running rrdtool 1.4.4, and PHP 5.3.3 on a 10.6.6 Server, and the render times for graphs via a web browser are horrible. rrdtool spikes the CPU and it probably lasts that way for 10-15 seconds.
This is a 2.66ghz dual core Mac Mini, 4GB of ram.
running rrdtool as root or _www returns results instantly (at least rrdtool info /path/to/file.rrd). Not sure how to generate a graph from the rrd file to test.
This is the only info I've found relating to this issue, so I'd be very keen on figuring out why performance is so pitiful.

I have found the solution, in Mac OS X and if you are using macports.
1. port uninstall -f fontconfig
2. port install fontconfig
This recreates the font caches in the right place. The problem is solved.
