I see in another thread that you've solved a similar issue in another thread (from april) with a change to <index_order> but I admit I don't understand that change. Any chance it may apply to me? I'm still trying to get a grip on what may be wrong here...

Ok, I was looking again at thread 36868, and I see that is not the same problem I'm having, as their issue was that they tried to index on output fields rather than on the input fields. My index is on input fields (5 of them).

This indeed solves your problem. Please use and retry.
You may want to download lib/data_query.php which now includes better checks for <index_order> and more. This will be part of 087h and hopefully help on debugging data query XML

I was having a very similar issue to this.
Once I checked the top three boxes in the Custom Data area of the Data Template I realized that those fields only got properly filled when the graph was created from the Data Query section of the device rather than the Graph Template section.
Once I made that change and starting creating my graphs correctly it started working for me.
