UCD/NET Available Disk Space

The UCD/NET Available Disk Space test requires three fields, which I do not know how to complete:

Create Graph from 'ucd/net - Available Disk Space'
Graph [Template: ucd/net - Available Disk Space]
Data Source [Template: ucd/net - Hard Drive Space]
Name - Choose a name for this data source.
Custom Data [Template: ucd/net - Hard Drive Space]
Output Type ID
Index Value
Index Type

Can somebody tell me how to fill out the three boldfaced fields above to monitor the root (/) partition?
