nmidphpip plugin download, where?

Hm, I'm not sure at the moment, why the tree is not filled correctly in your case, is the http error log telling anything unusual ?
For the prefixes being listed: That's by design, as you are able to edit the unallocated ips manually. If the unallocated prefixed are not listed, the only way to add allocated ips ( of devices NOT in cacti ) is via the mysql cli interface or the import function ...
The original code is from http://www.phpip.net/console.php and it had the same behaviour for displaying it

php error when clicking on nmidphpip menu
NMID phpIP - IP Search

This happens when I click on the nmidphpip link on the console section. Any ideas?

That's probably a windows specific issue. THere's a bug in the automated directory detection part which needs to be fixed.
