can not creat Threshold Template!

i install the thold

when i created my first threshold template,Threshold Templates -> add -> select a Data Template ->select a Data Field -> go, it display 'Error: A template with that Data Source already exists!'
and in my ''Data Sources' -> 'select a host' (on top) ,it only 'Template Name', not 'Template Name/ Click for THold'

what's wrong with my thold?
