this oid will give you the 5 minute running average

First add a new data source. Select Gauge as the Data Source Type, Get SNMP Data as the data input type and set the Maximum value to 100. Save it. Click on edit data for the data source just added and specify the IP Address, SNMP community and the oid string for the router you want monitor. Save that.

Then add a graph.

If you need more help, there is a section on adding data sources and graphs in the online documents.
